2025 Award Nominations

The Colorado Public Health Association (CPHA) awards have been established to highlight and honor the contributions, efforts, and achievements of the public health community in Colorado. Thank you for an interest in submitting a nomination for a Colorado Public Health Association Award!

Eligibility Criteria:

  • Any individual, team, or organization that has contributed, supported, or advanced efforts of public health and/or the public health community in Colorado. Additional criteria will be outlined in the specific awards.

 

Nomination Process:

  • To submit an award nomination, review the award’s descriptions and respond to the application questions below.

  • Nominations may be made by an individual or group. We also invite you to nominate yourself!

  • Nominations are due by: 07/16/2025

  • Once award recipients are selected both nominators and awardees will be notified via email or phone the week of August 4, 2025.

  • Award recipients will be asked for a photo to be used during the awards ceremony and post-ceremony announcements that CPHA will share through email and social media.   

 

Review Criteria: 

  • The selection committee consists of Colorado Public Health Association board members and Public Health in the Rockies committee leads. The selection committee will use a rubric to guide the review process to determine award selection and can be viewed here.

  • There are eight award categories, the awards committee will use experience and discretion to pair each nomination with the most fitting award category. This approach is intended to ensure that every nominee receives fair consideration and that the awarded recognition aligns most accurately with their contributions to public health. 

  • If you are interested in nominating someone for a specific award category, please specify and use the category description to guide your response. 

  • Tips for crafting a compelling nomination:

    • Provide concrete examples of the nominee's accomplishments and how they meet the award criteria. 

    • Highlight the nominee's contributions and the positive impact they have had on their organization or community. 

    • Use clear, direct language to make the nomination easy to understand. 

    • Proofread so nomination is free of errors in grammar and spelling.
