Colorado Minority Health Forum

Mission: The Colorado Minority Health Forum is an inclusive, membership organization that educates Colorado communities about the human cost of health disparities and advocates for the elimination of health disparities.

Vision: Health care providers, health care organizations and policy makers across the state of Colorado will understand the term "Health Disparities."  They will be aware that they are either contributors of health disparities or that they are contributors to the elimination of health disparities.  All will understand how the core causes of health disparities disproportionately impact communities of color.  All will establish community level accountability measures and indicators for the elimination of health disparities in communities of color and report results to their community annually.
